外周血免疫细胞变化与脑血管狭窄的关系研究

【摘要】 目的 探讨外周血免疫细胞亚群与脑血管狭窄的关系。方法 根据影像学检查结果将入组病例分为无症状组 1 163 例,轻度血管狭窄组 209 例,中度血管狭窄组 71 例和重度血管狭窄组 26 例。收集外周血进行免疫细胞检测,流式分析不同免疫细胞亚群与血管狭窄程度的关系及青中年血管狭窄的危险因素。结果 脑血管狭窄组外周血中 CD3+ (无症状 vs 狭窄组:67.58±0.32 vs 67.11±0.70)、CD3+CD4+ (无症状 vs 狭窄组:37.48±0.28 vs 35.64±0.51)T 细胞亚群、CD16+ CD56+ (无症状 vs 狭窄组:19.17±0.48 vs 17.40±0.52)的 NK 细胞亚群、CD19+ (无症状 vs 狭窄组:12.29±0.24 vs 10.28±0.36)的 B 细胞亚群比例均 明显低于无症状组,CD3+ CD8+(无症状 vs 狭窄组:26.58±0.28 vs 27.81±0.63)T 细胞亚群比例均明显高于无症状组,CD4+ /CD8+ (无症状 vs 狭窄组:1.64±0.03 vs 1.55±0.04)比值显著下降(P<0.05),呈现外周免疫抑制。青中年血管狭窄组免疫抑制更为明显。结论 血管狭窄水平与外周血中免疫细胞变化密切相关,免疫抑制是预测青中年脑血管狭窄的危险因素及检测指标。

【Abstract】 Objective To investigate the relationship between peripheral blood immune cell subsets and cerebrovascular stenosis. Methods According to the results of imaging examination,the enrolled cases were divided into the asymptomatic group(1 163 cases),the mild group(209 cases),the moderate group(71 cases)and the severe group(26 cases). Peripheral blood was collected for immune cell detection,and the relationship between different immune cell subsets and degree of vascular stenosis and the risk factors of vascular stenosis in young age group were analyzed by flow cytometry. Results The percentages of CD3+ ,CD3+ CD4+ T cell subsets,CD16+ CD56+ NK cell subsets and CD19+ cell subsets in peripheral blood of the cerebrovascular stenosis group were significantly lower than those of the asymptomatic group,and the percentages of CD3+ CD8+ T cell subsets were significantly higher than those of the asymptomatic group(P<0.05),showing peripheral immunosuppression. The immunosuppression was more obvious in the young age group with vascular stenosis. Conclusion The level of vascular stenosis is closely related to the changes of immune cells in peripheral blood. Prediction of risk factors and detection indexes of cerebrovascular stenosis in young-aged patients by immunosuppression.
