进展性卒中的研究进展

【摘要】 卒中是全球范围内第二大死因，且脑卒中已成为国内的第一位致死原因，其中急性缺血性卒中占 80%，据统计有大约 1/3 的患者会在发病 1 周内出现神经功能恶化，较其他卒中患者具有更高的致残率及病死率，这一类型卒中多归类为进展性脑卒中。进展性卒中是脑卒中患者预后不良的重要原因，提高早期认识对其预防及改善预后具有重要意义，目前进展性卒中国内外无统一的临床共识或指南，故本文就其定义、发病机制、危险因素、影像学特点及治疗进行系统综述，旨在探讨进展性卒中科学的诊断标准、早期识别信号、预防措施及治疗方法，以期降低脑卒中的致死、致残率，改善患者远期神经功能。

【Abstract】 Stroke is the second largest cause of death worldwide，and stroke has become the first cause of death for Chinese people，of which acute ischemic stroke accounts for 80 %.According to statistics，about 1/3 of the patients will have neurological deterioration within one week of onset，and have higher disability rate and mortality than other stroke patients.This type of stroke is mostly classified as progressive stroke（PS）.Progressive stroke is an important reason for poor prognosis of stroke patients.Improving early understanding is of great significance for its prevention and improvement of prognosis.At present，there is no unified clinical consensus or guideline for progressive stroke at home and abroad.Therefore，this paper systematically reviews its definition，pathogenesis，risk factors，imaging characteristics and treatment，aiming to explore the scientific diagnostic criteria，early identification signals，preventive measures and treatment methods of progressive stroke，so as to reduce the mortality and disability rate of stroke and improve the long-term neurological function of patients.
